Fujistsu DX200 S5 ISCSI connection

I am using Fujitsu DX200S5 connected to 3 Fujitsu servers via Fibre Channel ISCSI port. The storage is connected directly to each server without intermediary switch.
Everything looks fine, each server is connected to storage but time to time the partition which is connected to sever become hardly accessible and I do not know the reason. Can anybody help me with some advice?
